The Host Home model allows an individual to live in a private residence with a caregiver (non-related parent, guardian or spouse) rather than in an apartment or group home. Host Home Providers are not UVS employees; they are independent contractors and manage their own home business. Host Home families assist the individual in developing skills and building relationships in the community, the family, and their own personal life. Host Home living is highly flexible and focused on personal choice. Individuals residing in our host homes
receive companionship and assistance with:
• Medication administration
• Meal preparation
• Budgeting
• Laundry
• Community participation
• Recreational outings
• Advocacy and navigation of the DIDD
system and services
All individuals considering a host home setting will have the opportunity to meet several providers and see several homes before selecting their host home provider. All of our host home providers are trained by agency staff to provide the best care possible and to provide appropriate supervision as determined by the individual and their team.
Independent Living Services are provided to an individual living in his or her own home or apartment with or without roommates, depending on the location. Independent Living Services are provided to an individual living in his or her own home or apartment with or without roommates, depending on the location. These sites serve individuals of various need levels, including individuals with significant medical, physical, or behavioral challenges. Staffing levels vary by the needs of the people being served. The focus of these services is to enhance independence and quality of life. UVS Independent living services are very flexible and easily modified to meet changing abilities, interests, choices, and needs of individuals.
Individuals in Independent Living are able to function independently by cooking, cleaning and caring for their personal hygiene with little to no assistance. A majority of our customers have jobs, go out with friends and family, go to libraries, stores and enjoy taking part in their community. Staff meets with each individual for 2 to 12 hours per week to help with daily needs, laundry, cleaning, cooking, and shopping. Staff also fill a weekly medication reminder box for daily medications. Our IL program also supplies Medical Support staff to schedule and transport individuals to various medical appointments, as well as keeping track of all regular medical needs.
Under the Family Care Giver option, individuals receive their services directly from a family member instead of an unrelated staff. This may include assistance with tasks such as bathing, dressing, medications or housekeeping. The amount and type of involvement are determined by the individual and their team. similar to Supported Living
Under the Family Care Giver option, individuals receive their services directly from a family member instead of an unrelated staff. The amount and type of involvement are determined by the individual and their team. similar to Supported Living Services (SLS), areas of paid support may include assistance with bathing, dressing, meal planning, budgeting, housekeeping and other assistance regarding activities of daily living. The level and type of support are established by the individual and their team members. The IDT must agree that the Family Caregiver option is the least restrictive for the individual. This means the individual's life should resemble those of other people the same age.
The individual's comprehensive funding remains the same as residential, day program, and transportation services. The number of paid hours are agreed upon and dependent on the funding the person has. Other hours are considered natural supports. The services provided must meet the agency and state standards which are the same for group home and host homes. The home must pass a HUD inspection. If the family members want to be paid, they will need to become an UVS employee. Background checks and training requirements must be met. Another option is having an UVS employee provide the residential service in the home with the family providing natural supports. Residential program staff will monitor the home for quality of services at least monthly.
